You choose option one and ended up turning Jeremy into the younger sister.

Aftermath:
Jeremy became Jenny, she also became 16 years old while his younger now older sister is 18. They share clothes and Jenny believe she's always been a girl and never a guy. She now has a crush on her old Best friend, and Jenny  became BFF with her older sister.
